PVCs: There are pvcs associated with slow heart and with rapid heart, it isn't necessarily an evil implication with either situation. Need more info: An ECG or a holter study may show whether you have pvcs. Isolated pvcs are not necessarily concerning if the heart is otherwise healthy. These are usually suppressed by tachycardia (e.g., during exercise). However, if the extra beats are increased during exercise, this is potentially concerning. It is highly: unlikely that you have had structural change in six months in the absence of trauma, infarction, infection or toxic drug exposure. Orthostatic hypotension with physiological sinus tachycardia may represent autonomic issues which are cyclical. This can also account for your holter findings. Maintain good hydration.
