pneumonia, pulled muscles off the ribs, a rib fracture, bruising of the chest wall, pain from the back (spine), kidney, spleen or pancreas problems, etc, etc. Rarely a neuroma (a nerve lesion) in the skin.
You don't need to go the er but you should see your pcp or a free clinic to be evaluated.
