I assume: You mean swelling the right lower abdomen. Often this could be a symptom of bowel gas or constipation. If there is pain, tenderness, nausea, vomiting, fever, loss of appetite, etc. Then appendicitis can cause those symptoms. Not all appendicoliths result in appendicits, but no one knows how many. Yes: That's how it happens sometimes. There is an abscess in the appendix, and it grows bigger, swelling and causing pain. Then the abscess leaks out of the appendix and the pressure inside decreases (called a burst appendix), causing less pain.
