What Lab Work Is Needful Before Starting Remicade Injections
On remicade, (infliximab) lab tests show low pneumonia, diphtheria, and tetanus antibodies? Should I do anything or be concerned?
When is it time to start Humira or Remicade (infliximab) for RA. Been on MTX & plaquinil for 14 yrs, pred on & off. Labs slightly off, but pain & swelling intense.

Last labs showed high iron saturation at 55%, normal total iron, and normal iron binding capacity. Low wbc. Should I be concerned about these? Currently on plaquenil, sulfasalazine, and Remicade (infliximab) infusions for Lupus/RA.
How low?: It depends on how low the WBC is and whether this has been a persistent finding. You will need to find out the cause for the low WBC which may be the result of a drug reaction or just part of the Lupus/RA. Resistant scleritis: Initial therapy always requires systemic nsaids, glucocorticoids, or other immunosuppressives (rituximab & cyclophosphamide). Cyclosporine & mycophenolate may also work. Azathioprine or mtx may be most beneficial for maintenance. Tnf Alpha inhibitors (eg infliximab) may be effective in resistant disease. Probably: You are at an increased risk of infection overall while on remicade (infliximab). Using a strict sterile technique and wearing a surgical mask as a doctor can decrease this risk substantially but it will not be 0. Talk to your pain physician about it. ...Read more

Depends. : Depends on the examining doctor who is taking care of his medications. As u may have been explained Remicade (infliximab) may allow infections to become life threatening. If IV antibiotics are contemplated they can be administered at home with an indwelling peripheral vein catheter and nurse home visits. Sometimes just drainage of abscess is all that may be required. If : If you have type 1 diabetes, why not take a dose of fast acting insulin? Humalog, apidra, (insulin glulisine) and NovoLog work within 15 minutes to start lowering the sugar. I do not know of any clear glucose level at which you could not get remicade. Makes little sense why there would be since glucose can vary with Insulin and over hours. Your rheumatologist may not be comfortable dosing your Remicade though if your sugar is high. Contrary to popular belief, drinking water will not lower your sugar. Switch.: Remicade (infliximab) blocks the tnf-a receptor. Drugs like Humira an Enbrel are antibodies to tnf-a. I recommend switching to one of these (I favour humira), since they work differently. Humira is the only one of the 3 that is fully human. ...Read more

Fine: A very high percentage of patients I treat with Remicade do fine during the infusions and after. If we do get complaints from patients the day after a Remicade infusion they include headache usually controlled with tylenol (acetaminophen), (acetaminophen) fatigue which may last a day or two. Please see PCP: He may be dehydrated, which would cause "orthostatic hypotension" as previously described. However, if this does not go away with hydration, he may need to see an ophthalmologist, as there are eye conditions associated with crohn's disease.
