Methotrexate injections and pills didn't work for scleritis and was wondering if remicade (infliximab) is good medicine for the scletitis?

Resistant scleritis: Initial therapy always requires systemic nsaids, glucocorticoids, or other immunosuppressives (rituximab & cyclophosphamide). Cyclosporine & mycophenolate may also work. Azathioprine or mtx may be most beneficial for maintenance. Tnf Alpha inhibitors (eg infliximab) may be effective in resistant disease. Scleritis should ideally be managed by ophthalmologist+rheumatologist experts. ...Read more

100mg minocycline 1 time a day is starting not to work for acne anymore after 3 years, would upping the dose possibly help? Any other options?

Probably no.: The P. acnes bacterias has probably become resistant. 3 years is a very long time to be taking Minocycline. It has too many side effects, with long term treatment may cause autoimmune reactions (systemic lupus erythematosus, autoimmune hepatitis). Other alternatives are topical antibiotics like Epiduo, Benzaclin, (clindamycin and benzoyl peroxide) etc, laser treatments can be very effective. ...Read more

My anti ccp is 300 , am taking 17.5 mg mtx and will start soon a combination of enbrel (etanercept) and mtx is that good ?

Combination therapy: Rheumatoid arthritis should be diagnosed and managed by a trained rheumatologist. The anti-ccp antibody is highly specific for RA. Methotrexate, in combination with other medications, can often successfully treat RA and prevent joint damage. Sometimes, MTX is combined with sulfasalazine and hydroxychloroquine. Alternatively, a "biologic" agent is added such as Etanercept (Enbrel), which blocks TNF ...Read more

After IV iron tx for IDA, what retic count would reassure that the tx is working?

After IV iron tx for IDA, what retic count would reassure that the tx is working?

Dynamic response: It is a dynamic response (not exact number). Usually, you see retic count begin to rise at day 3, maximize in 1-2 wks. Hb begin to rise in day 7-10, can increase by as high as 2gm at 4th wk. Reticulocyte hemoglobin content and % of hypochrome reticulocyte are mush suprior than absolute reticular count in predicting bone marrow response to iron therapy. ...Read more

Is it necessary to take azathiroprine while taking remicade (infliximab) infusions?? What do the two drugs do to help you? Or help each other.

It depends: Azathioprine can reduce the rish of infusion reactions to Remicade (infliximab) and can inhibit the production of antibodies to Remicade (infliximab) that can compromise its effectiveness. However, it is not absolutely essential to take it while on Remicade (infliximab). ...Read more
