I recommend physical: medicine & rehabilitation. A physiatrist can diagnose and treat all kinds of physical problems causing neck and upper limb pain. Treatments include, but are not limited to, myofascial release, trigger point injections, epidural steroid injections, physical therapy, bodywork, biofeedback and manual therapy.
