Probably a fungus: Fungus spores are in the air and can get into the nail due to trauma. The nail grows thicker, discolored and may have an odor. There are over the counter remedies, If these don't work, see a podiatrist for Rx medication.

As a toenail grows out the fungus attaches to its distal edge and move proximally or toward the body. If you can't get it out on your own, a doctor can. Sometimes this can be done without numbing the finger, but often, anesthesia is necessary. If it is a chronic condition, there is a very successful surgery that can be done to permanently correct the problem.
