Caseating granulomas: This sounds like tuberculosis, but may conceivably be systemic fungal infection like Histoplasmosis, rarely lymphoma or sarcoidosis. What exactly is a CSF culture & what is its purpose? Is it difficult to perform and does it take longer than blood cultures for a result to come back?

Similar to blood: A CSF culture is when you take a sample of CSF (usually from a lumbar puncture which samples the CSF from the spinal canal in the lower back - so yes a bit more difficult to perform but still pretty routine) and culture it to see if there are bacteria (or sometimes viruses) growing in it, usually to look for meningitis. It takes the same amount of time as a blood culture (48 hours). ...Read more

Can you think of any reason why blood cultures would come back with negative results within 2 hours even though antibiotics not commenced until hours after the results have come back.

Unclear question: If the blood culture results were reported within 2 hours, then it could not have bee culture, as that takes at least 24 hours, longer depending on the type of organism and microbial load. It was probably some other test like CRP, not blood culture, that was reported within two hours of blood collection. It may help to talk to the pathologist in-charge of the laboratory. ...Read more
