I get a hot (not itchy) red rash over my face (ruled out eczema & rosacea) that comes & goes sometimes quickly. Dr's don't know, sed rate test was ok.

Why would a dermatologist assume a Malar rash is Rosacea and not Lupus because it is not a raised rash? I have read Lupus can appear as flat lesions?

"adult acne"..... not an uncommon skin disorder its distribution is more widespread...the treatment options are many. In women, hormonal studies are probably indicated (for example Polycystic ovary Syndrome patients have a higher incidence of adult acneiform skin lesions)
