Treat Immediately: Treat pid as soon as it is diagnosed. There is nothing in the literature that mentions how long pid can go untreated without complications. I would guess if untreated, depending on the specifics (infecting organism, extent of infection, patient's immunologic response etc.) the complications may already be in place at diagnosis or within a few days of diagnosis.
