Treatment is correct: What you are describing is a post op hematoma. You are right in that a blood vessel bled, resulting in an accumulation of blood(hematoma-knot under the incision). Some blood spread down under the skin into the vaginal area to discolor the vagina(black). This is called ecchymosis.
