When patients have severe urticaria, we often have them take even more than 4 zyrtec (cetirizine) tablets daily. The initial studies with zyrtec (cetirizine) used doses of 10-40 mg, or up to 4 tablets at a time. You can still take the ranitidine and montelukast at night also, if they help you. If not wanting to take Allegra twice a day, and wanting to add a different antihistamine such as Zyrtec or Xyzal (levocetirizine), (levocetirizine) it is probably enough to take Zyrtec or Xyzal (levocetirizine) (one of them instead of both of them). Zyrtec and Xyzal (levocetirizine) are very similar. The makers of Xyzal (levocetirizine) say it is more effective than Zyrtec, but they are similar drugs. ...Read more

Wait: Medication can lose it's effectiveness past expiration date, but I would not expect it to not be effective at all; especially if only 18 months expired. It will not be harmful at all that you took an expired zyrtec.
