If you are taking a product that contains creatine in addition to other supplements, perhaps one or more of the other compounds may be the issue. More likely muscular: If by "kidney area" you mean mid-back to one side or the other, I would think that it is more likely that you're feeling symptoms related to the musculature of the back, e.g., muscle spasm, than due to the kidney. Kidney's can hurt, when swollen or inflamed, e.g., when infected, but this is less likely to be an intermittent problem. A simple urine test can rule out infection. ...Read more

Elevated BUN/cr: Would not worry. Likely due or dehydration as most people do not drink enough fluids. If other labs like creatinine alone are OK (which it appears to be as BUN is 28 and the ratio is 30 so creatinine has to be < 1) and liver functions are OK, don't worry. BUN is slightly higher in older individuals.. Good luck with surgery.

Not advisable: While there are no contraindications to these drugs combined, it is not a great idea to stress your kidneys so much. Accutane can cause kidney problems as can creatine, so combining the two will likely increase the risk, even if you drink a lot of fluids.
