Severe pain, an emergency, is called ovarian torsion where a heavy cysts twist on the blood supply causing what we call an "acute abdomen". Lower abdominal pain is more broad. I was diagnosed with a left ovarian cyst in december. I have had left ovarian pain since october, now the pain is almost constant and pulling. Is it possible the cyst is still there a fairly large?

Generally No: Ovarian nerve supply that relays pain is through the sympathetic nervous system between t11, t12 and L1 levels. This is obviously above the L4 and L5 levels you stated, however pelvic pain could still be related to non-ovarian causes and if you had significant pathology at l4/l5 it could be a source of this pain. Consider: It may be a urinary tract infection (which could cause pain the lower pelvis that could feel like ovarian pain) or a kidney stone.
