You could be having premature beats, mostly pvcs or pacs. From heart the blood runs to the brain through the neck and many people having pvcs they might feel palpitations in their necks and the ears.

If you've only recently started on these medications, it is possible that you are experiencing some tinnitus as a side effect, most likely to the venlafaxine. What: What you are describing sounds like pulsatile tinnitis. Before recommending treatment, it would be reasonable to reconnect with your ENT to explore possible causes that include certain vascular tumors of the head and neck (i.e., paragangliomas), atherosclerosis that may be creating narrowing and turbulent flow of your carotid artery (which runs close to your middle ear), possible avm (arteriovenous malformation), etc. After a thorough head and neck exam, and audiogram, he/she may wish to get a contrast ct or mri/mra study.
