Any teenaged boy with good muscles will have creatinine 1.1 or higher; in whole classes of athletic med students, most men had creatinine above this. High creatinine: Serum c is dependent on the person's weight, race and age. You need to have a 24 hour urine collection for a creatinine (c) clearance determination to know what your renal function is. When the urine is returned, blood is taken for a c level. Big picture: If you're fairly muscular, a 1.7 creatinine is probably normal for you; when I was lifting weights every day, my creatinine level was 1.7. I trust that your diagnosis of gout is correct and that you are taking maintenace medication for it. If so, uric acid nephropathy is unlikely. For an actual gfr, you need to have a 24 hour urine collection, along with some blood work to see if you have any disease of the kidney. No simple answer: These values could at worst be an indication of multiple myeloma. Or more likely, she has some kidney impairment due to age and high blood pressure and perhaps the elevated calcium is due to her hydrochlorothiazide.
