May be endometriosis: A number of causes for example: endometriosis, dysmenorrhea, fibroids, bowel or bladder disease. You need to discuss this with you primary care doctor or gynacologist, for proper work-up and treatment. Proper treatment will depend on your exact diagnosis, some will require nsaids, ocps, or other therapies depending on diagnosis.

Monthly ache: Hip pain is usually felt in the groin, so it does not sound like you are experiencing pain from your hip. Both back and muscle pain can get worse with your menstrual cycle- it is worth discussing with your pmd- with possible referral to a physiatrist.

Need to examine: It is not feasible to know why your labia are itchy, without examining you. You may have yeast infection or a skin disorder. It would be prudent to see your doctor.

Very common: Varies with each individual person and depending on what you eat which can affect the degree of above symptoms you just described. If persistent and concerning, consult with your physician.

See details: At age 49 you are likely peri-menopausal so period variation is to be expected. The pain is not GYN but could be GI in origin. See a gastroenterologist

Back+Leg pain: A combination of back- and leg pain and cramps could be due to an acute disc herniation or another cause of nerve compression. Have yourself seen by a qualified doctor and maybe have an MRI outlining the nerves and your lumbar spine structures.... Good luck!
