May be endometriosis: A number of causes for example: endometriosis, dysmenorrhea, fibroids, bowel or bladder disease. Need to examine: It is not feasible to know why your labia are itchy, without examining you. You may have yeast infection or a skin disorder. It would be prudent to see your doctor.
Just diagnosed w/uterine fibroid (s). Don't know if it's one or several. Severe low back pain, menses for 25 days. Are Fibroids enough to cause this?
Possibly: Ask person who made the diagnosis. Get a sonogram to see how many.

See details: At age 49 you are likely peri-menopausal so period variation is to be expected. The pain is not GYN but could be GI in origin. See a gastroenterologist
