Most likely reflux: Most likely cause is acid reflux even if there is no heartburn. Only 1/3 of people with reflux have heartburn. Consider avoiding caffeine, alcohol, acidic or spicy foods, smoking, eating less than 4 hrs before sleep, sleeping with the the head higher than the heart, and antacids. Swallow study: You should have a proper swallow study done. The wrinkles in the skin of your neck have nothing to do with difficulty swallowing or a choking sensation as you describe it.
