See below : Mainly due to muscle stretch/strain. Also changes in the way you walk due to changes in your body and your center of gravity.
Center of gravity: Most likely, her changing center of gravity (enlarging pregnant uterus) is pulling her back forward and causing a strain. A belly sling can help lift the uterus off the pelvic and may give some relief. A search for "belly sling" on Amazon.com will show you some examples. Floating in a pool can also bouy the baby up off the pelvis and can help with low back pain & swelling.

Generally the initial treatment for this is conservative - physical therapy, increased activity, maintaining a good diet and weight, smoking cessation, etc. Abdominal cramping is not typically associated and may be separate.
Get evaluated by doc: Occasional ache/pain occurs in everyone at some point, but persistent pain should be evaluated. RFA, PT, NSAIDs: Curved spine (scoliosis) and "worn discs" are extremely common in people that have no pain at all, so both of those things aren't necessarily the causes of pain. Both though, can cause pain by putting pressure on the joints in the back. A thorough evaluation by a spine specialist should be able to tell you what is causing the pain (the joints, the discs, the muscles/ligaments, etc).
