Shouldn't: Cirrhosis causes splenomegaly through portal hypertension and splenic congestion; a mildly fatty liver should be well before this stage, and should not be the cause of a painful enlarged spleen. You should see a doctor for further evaluation and testing. ...Read more

The spleen does not enlarge in alcohol-related disease until the liver is very sick (cirrhosis).
