Fluctuating hcg: Fluctuated levels of hcg is not a reassuring sign. The simple fact is that just because the fetus is in the uterus does not mean she is not miscarrying. Sometimes a spontaneous abortion is followed by spontaneous delivery of the fetus but other times the fetus must be delivered by the doctor. You don't expect to see a sac until the HCG reaches 1500 to 2000, and you won't see an embryo for at least another 1-2 weeks, and then just a small blip on the screen. Hcg levels: Rising hcg's are reassuring. However, I usually want to see a Progesterone level in patients at risk of ectopic, especially if plateau of HCG is noted. Most (but not all) ectopics have very low Progesterone levels. Often times even lower than an early miscarriage. See if your OB is comfortable with ordering and interpreting Progesterone levels. ...Read more
