Many things: The list is long but redness, tenderness, effected area warmer than rest of your body, swollen: these all can indicate infection whether it be just under the skin or deeper or phlebitis to even a blood clot in underlying tissue. You should be applying a topical antibiotic ointment, like neosporin, to help the wound heal and also reduce risk for infection. You should also keep it covered with gauze.
