If you step on a sharp metal and there is no cut and the skin is intact. You do not need Tetanus shot.But if there is an entry wound (the metal entered the skin)then you do not need a Tetanus shot if your last booster shot was more than 5 years ago

Tdap recommended: Tetanus shots are good for 10 years but if the injury is 'dirty' then >5 years is the norm. However, the adult tdap is recommended now to cover tetanus and whooping cough. The other worry with a puncture wound is the risk of infection. I hope you could clean the wound with soap and water. It is possible that you have a secondary bacterial infection from the puncture wound, and may need antibiotics. Tetanus is very unlikely, and wouldn't present this way. But you should make sure that you are up to date on your immunizations-- tetanus is called Tdap-- adults need a booster every 10 years.

No: The skin must be punctured completely to force the clostridium tetani bacteria through to underlying tissues order to cause tetanus. A scrape won't introduce it, as well.
