IF: menstrual blood is black - don't worry. That just means that old blood is being cleaned from the uterus. If your bowel movement is black - that might be from taking peptobismol or iron. Every woman is different. Typically a woman bleeds for about 6 weeks after childbirth. However, the fact that you are changing your pad every hour is concerning due to the fact that you may be anemic. Regardless, your doctor needs to be informed of the heavy bleeding. He/she may will likely want you to come in for a pelvic exam and testing for anemia.

Depends: Where are you using the heat pad? If the back, then 20-30 minutes but not at scolding temperatures. If it's the jaw, probably not a good idea if you just had surgery.
