VWD: First of all, these test need to be repeated at least one more time, prederably while you are off all medications. If persistently low, this may mean that you have von willibrand disease (VWD) which is a bleeding disorder. These are several types of VWD and more testing is needed to determine the type. Would normal CBC, metabolic panel, ESR, CRP, PT, aPTT, d-dimer make multiple myeloma or waldenstrom macroglobulinemia unlikely? Given a presumptive diagnosis of von Willebrand at age 31 with no known family history. Concerned it may be acquired form.
Not right tests: If you wish to be tested for multiple myeloma or Waldenstrom, you should have serum protein electrophoresis and immunofixation electrophoresis and if needed similar studies on urine. Von wiilebrand factor is a clotting protein that helps stabilize clots by helping to bind platelets together and also helps carry factor viii in the blood. In vwd, vw factor is either decreased or does not function appropriately. Different but....: Related. Vwf is a glycoprotein important for primary hemostasis. It cross links collagen at a site of injury with gpib on platelets. Deficiency leads to failure of primary hemostasis (ie, bleeding). Vwf also protects fviii from degradation. Fviii partners with fix to activate fx to ultimately promote clotting. Supposing that your girl friend has one VWD on one chromosone, and the other is normal, the chance of her passing that VWD gene to your offspring would be 50%. Why?: I agree that there are probably others out there with the same combination of problems. And this should not preclude any necessary therapy. If you have bleeding due to your vwd, treatment can be provided without greatly increasing your risk for developing a clot. Stimulaton test: Patients with type 1 VWD less vw factor than normal people. However, they store vwf normally in the cells that line their blood vessels. This can be released into the blood by certain drugs; one of these is ddavp (desmopressin). In a DDAVP (desmopressin) challenge the vwf level is measured, a dose of DDAVP (desmopressin) is given, and the level measured again to see if it goes up significantly.
