The problem here: Is you need to know if the toenail became fungal or if that nail is thick for other reasons..I.e bacterial colonization or subungual keratinazation. A thick yellow stubby nail is not always fungal.
Topical meds for fungus are not highly effective either. If your inclined to try one you can but you will be applying for many months duration & 1 is not necessarily better than others. ...Read more

Local infection?: I wouldn't think endocarditis based on that. But it could be a local infection. When we cut nails too short; or pull off too much, we can tug some skin out with the nail, leaving a cut that can allow for the introduction of bacteria. ...Read more

Absolutely OK: It is very safe to build muscle.
If you haven't exercised in a long time, then work into it gradually.
If you are lifting at a gym, work with others and they should help you gradually increase your weights and reps.
If you are lifting at home, then ask a friend that lifts or google a program.
Mildly sore muscles is a sign of a good workout.
Use "day-on, day-off". Arms one day, legs the next. ...Read more

heels: I feel you should only use shoes to raise your height. Too much mass in your shoes will cause problems for your toes, top of your shoes or blisters to your heels. Make sure you keep your shoes at the same height to prevent any back issues. ...Read more

New socks, shoes?: My personal experience having run dozens of marathons (five bostons) is that any recurring pain on the top of the foot may be the result of running shoe wear or possible "bunching" irritation of socks. Generally surgery involves removal of a wedge of bone which would shorten the bone, but the once the correction of the angular deformity is done, it more than compensates for this.
