Not great combo: Concomitant administration of clonazepam (Klonopin) and carbamazepine (Tegretol) resulted in a 22% increase in clonazepam clearance and a 20.5% decrease in carbamazepine clearance. Carbamazepine is well known to induce its own metabolism as well as the metabolism of other meds making it difficult to manage. Consult provider. Not fun: Usually the strategy is to slowly taper off the benzo and use a long-acting one, and sometimes it's really, really slow. Here's a trick i sometimes suggest to my patients: when your dose is pretty low, gently drag you pill across an emery board before you take it. OVERLAPPING MEDS: The combo of Meds you are on looks like you are being treated for anxiety and depression . Xanax (alprazolam) is short acting for anxiety . Wouldn't recommend: you are already on several drugs which inhibit serotonin reuptake, increasing their levels - amitriptylene, Cymbalta (duloxetine) and cyclobenzaprine all have that potential, and ondansetron does it big time. This can result in the serotonin syndrome which is dangerous and even potentially fatal. All of these drugs also produce sedation. Is it safe (non lethal) to have 2 drinks with 0.5mg Klonopin? I've done it several times. Is Valium safer than say Xanax (alprazolam) w/regards to having 2 drinks?
None are good: None of these are good to mix w/ alcohol. It may never kill you, but it's not safe and depending on other variations in your input (diet, illnesses, state of general health) it may suddenly be more risky. Not very common: A paradoxical reaction to a med is one that causes the opposite of usual expected effect. In a benzodiazepine it would be anxiety, insomnia, over-stimulation or agitation. If this occurs, one would suspect an unidentified underlying problem as associative cause (such as bipolar disorder, thyroid disease, drug abuse). Minor: Grapefruit juice inhibits the liver enzymes that break down certain medicines, resulting in a small increase in the medicine's effect. This definitely occurs with valium; there is some uncertainly whether it affects xanax (alprazolam) as much, or at all.
