Uterine Perforation (Overview)

When performing a uterine procedure through the vagina, a complication can be a uterine perforation. This is when an instrument is inserted through all the layers of the uterus, creating a hole in the uterus. Most perforations occur in the midline away from large blood vessels and usually can be observed. Lateral perforations or injury with an active energy electrode requires abdominal exploration.

Good Question!: Most perforations occur at insertion & can go unrecognized. However, in the last 5 years, I've seen several Mirena (levonorgestrel) iuds with normal follow up exams, normal placement verification (by exam) inexplicably be "lost" & then a negative ultrasound followed by an x-ray locating it in the abdomen.....
