Physiology: I would ask that you go see a physician who specializes in urology. Your descriptions make no sense and are not normal for a man. The only scenario where a non-erect penis is leaking fluid would be an std like gonorrhea or chlamydia or if the prostate gland is inflamed such as in prostatitis. An enlarged prostate with staining of underwear could be due to prostatitis. Is there discomfort on urination? What did the urine test show? Be sure to get a urinalysis with microscopic exam and a urine culture. Get a PSA test to rule out prostate cancer. If these don't give the answer, get a cystoscopy and possibly catscan.

Vaginal odor?: Panty shields are a choice, but aren't necessary. While there is a "smell" to a vagina, this is nothing to be ashamed of or try to cover up. It's normal. No advice!: Sleeping with or without underwear is strictly a personal preference. There is no reason to wear underwear (if you choose) and no reason not to wear underwear (if you choose). At times it is advantageous to not wear underwear in order for the perineum to be drier and cooler. If cold, certainly underwear is an advantage.
