Hern l5s1.Leg&back pain.Foot drop.Failed s1nerveblock.Emg&conduct study with nuero&brain scan clear.The drop is unexplained.Any suggestions would help?
?second opinion: Your disc problem is a little low to explain foot drop, unless a disc fragment is lodged at l-5. If this involved peripheral peroneal nerve or nerve root, would usually be picked up on emg, but perhaps you have spinal cord extrinsic compression at some level, as another explanation. Mri of neck or mid-back should demonstrate whether this is an issue. Have a conference with your doctors. ...Read more

My friend mmt(methadone) dr(not psychiatrist)rx him citalopram&s-valporate for uni-p mdd, its good rx?My psych advices bupropion on phone.Best regards.
Interesting issues: If you feel your friend needs to see an actual psychiatrist, you can encourage him or her to do so. If by "psych" you mean psychiatrist, you're putting him/her in an awkward position by asking for some kind of second-guessing on a patient s/he hasn't seen. If the "psych" (could also mean psychologist) suggests med changes anyway, it's ill-informed. His/her comments could also be misinterpreted. ...Read more

Hard to say: Unfortunately without true consultation, it would be hard to answer this question without causing harm to your father. These medications are very powerful and can result in extreme sedation, slowing your breathing down and even death. I would recommend a consultation with your pcp to assess further. ...Read more

My son w/ADHD can't tolerate Vyvanse or Concerta (irritability/rages) or Clonidine (extreme fatigue). What to try next? Straterra? Intuiv? Focalin (dexmethylphenidate)?
Testing for meds: A small percentage of patients with ADHD will be non-stimulant responders but I question the diagnosis when I see an atypical response to stimulants. Ask your MD about a MilleniumPGT saliva test to evaluate your child's genetic responses to a variety of psych meds to help narrow down the next choice. http://www.chadd.org/Understanding-ADHD/Parents-Caregivers-of-Children-with-ADHD/Evaluation-and-Treatment/ADHD-Medications-An-Overview.aspx ...Read more

Wrist pain: Without an mri- it sounds like wrist tendonitis with or without compression neuritis. I would consult an orhtopedic surgeon. Sometimes an injection of xylocaine (lidocaine) with a steroid will alleviate the pain. Also i always suggest the use of a pull on, neoprene tubular wrist support worn over a terry tennis wrist towel: it will really help. ...Read moreSee 1 more doctor answer
I was on Ritalin 120mg day, developed tolerance. Now Adderall 120mg, but unhelpful & side effects. Would Focalin (dexmethylphenidate) be better? & what dose comparable?
See below: Focalin (dexmethylphenidate) XR might feel better than Ritalin for some with ADHD. But tolerance to a medication typically requires being off. Dosing is best left to the psychiatrist who can evaluate you as it will also need to include weighing tolerance, symptoms and side effects. ...Read more
That depends: No diagnosis listed - just irritability, hopelessness & sleep disturbance. If the seroquel (quetiapine) was used solely for insomnia - I would stop that first. Seroquel (quetiapine) is a big gun for treating only sleep disturbance. A person could have severe depression & develop psychosis. If that were the case, I would stop the antipsychotic once the psychotic symptoms resolved - but would keep them on the antidepressant. ...Read more
