Required by law: It is required by law & insurance contracts that they have with companies & that you signed with those same companies. They need to make sure you are who you say you are & that they have proof. Also, if there is any legal question in the future, they can prove that they verified your identity. Don't think so: Not sure i fully understand but in general i'd say health care decisions are best made with your doctor(s) and family members-the people who have your health as their priority. Insurance companies often have profit for the company as their first priority. Medical Necessity: What this means is that it is necessary for a condition you have. With dextroamphetamine I'm assuming you use it for ADHD. Your insurance company must see an official diagnosis from your doctor. Usually they may require specific testing to diagnosis you.
