Yes there is a cost in time & money, both spent to benefit you - a worthy endeavor.

Medical Necessity: What this means is that it is necessary for a condition you have. With dextroamphetamine I'm assuming you use it for ADHD. Your insurance company must see an official diagnosis from your doctor. Usually they may require specific testing to diagnosis you. The insurance companies are tightening up and trying to prevent use and pay for medications and test. Hopefully this answered your question.

Basically, the diagnosis is via opthalmoscope or similar instruments.

If you mean a crossed eye, or an eye that is not straight, then it really depends on the cause. Sometimes glasses will help, but more often surgery is needed. A good exam with a pediatric ophthalmologist is important.
