Yes: If you have an underlying painful neck condition, i.E degenerative disc disease/pinched nerve/disc bulge; performing aggressive trapezius strengthening exercises can aggravate it. Try and do strengthening exercises with out weights. Consider working with a knowledgable physical therapist to make sure you are doing the appropriate exercises. That may be the most simple answer. If you are right handed, you perform more movements, carry more things, and generally work out your right arm and shoulder more than your left, which could easily account for some muscle asymmetry, right side larger than left.

Diagnosis: It really all comes down to the specific diagnosis. Long term narcotics would not be a good idea. Most common cause is an undeyinh structural problem in the spine such as a "pinched nerve" or a symptomatic facet joint. Other possibility us cervical dystonia. Yes: It can. Flexeril is a muscle relaxant but not a good pain reliever. You should rest, apply heat, get a message, have physical therapy, consider Yoga, and OTC analgesics may be helpful. You list Advil (ibuprofen). If that doesn't help, try Naproxen 2 tabs twice day with food. (caution: can cause stomach upset). Time is your best ally. ...Read more

If you mean the pain was underneath your scapula, the muscle underneath it is called the subscapularis. Tears involving this muscle or its tendon are detected using the lift off test.
