Transabdominal Cerclage (Overview)

A transabdominal cerclage is when the cervix is stitched closed but is done very high on the cervix through an abdominal incision. It is done for women with cervical insufficiency who have either failed two or more previous traditional cerclages (where the cervix is stitched shut from a vaginal approach) or when a transvaginal cerclage is technically impossible to perform. The surgery is performed before pregnancy occurs or during the early part of pregnancy, at 11-14 weeks.

Not really: A transvaginal approach called a mcdonald cerclage is the most common and most studies technique. Abdominal cerclages are reserved for those who fail a mcdonald cerclage.

How soon can I get a transabdominal cerclage put in after a d & c at 21 weeks.

Months: If your doctor is sure this is what you need, then I would probably wait 2-3 months for your uterus to recover from the pregnancy loss and shrink down to it's normal size and vascularity prior to undergoing an abdominal cerclage. There are some specialists who can do this procedure laparoscopically, which may be less invasive, and with a faster recovery. Good luck to you in the future.

Not necessarily: Most experts recommend reserving this procedure for patients that have cervical insufficiency who have had either failed 2 or more cervical cerclages or in a patient where a cervical cerclage cannot be placed (ex: significant shortening, scarring, or cervical laceration).

Can a transabdominal cerclage be performed before pregnancy?

Yes: Transabdominal cerclage is more risky than the typical cerclage done through the vagina, and women who need one cannot always find a local surgeon with enough experience to perform the procedure safely. It is best for a woman to speak with a local OB and find out if any cerclage is needed and who are the most experienced physicians to determine if this one is indicated and to perform the procedure.

Very rare procedure: Transabdominal cerclage is a rare procedure. Usually done for pregnancy related cervix insufficiency (or failure) in a patient with a failed regularly placed cerclage with the current or prior pregnancy. If this is your situation, you should be able to get a referral to a high risk obgyn close to you, from your regular doctor or your local hospital OB department. Good luck.

Cerclage: I would go to a Perinatologist in Houston first of all to determine if a cerclage and in particular, a TA cerclage is indicated in your situation. I'm not aware of anyone in particular in the Houston area that does TA cerclages. The top people in the country that perform TA cerclages are: Dr. Haney in Indianoplis ;Dr. Summerland in Chicago and Dr. Davis in NJ. I'm sure that there are also others.

Can mcdonald cerclage fail?

Yes: Transvaginal cerclages can fail for many different reasons. These types of procedures are reserved for women that have been diagnosed with cervical insufficiency. If you have experienced a pregnancy loss in the second trimester and if you have failed a transvaginal cerclage discuss with your doctor the possibiity of a pre-conceptual transabdominal cerclage (tac) for the next pregnancy.

Cerclage: As with any procedure there are risks. As procedures go a cerclage is at the lower end of the risk scale. Be sure to discuss with your OB.

Your OB doc: Or the perinatologist should have already gone over a detailed plan with you and your wife, so you really need to call your doc and discuss everything asap, but most likely (of course u will have to ask your doc) bedrest, probably im Progesterone to prevent premature labor but obviously unfortunately she is at increased risk of losing the pregnancy or delivering way early, hope it works out.

Should have pleasure: Should not affect the cerclage. Rear entry for vaginal penetration whilst spooning is probably sexual position of choice at this stage.

Can cerclage be performed under regional anesthesia?

Best option: Regional anesthesia (spinal)is the good option and frequently used during cerclage. Very little medication is used, which will unlikely to affect a baby. If appropriate dose is used recovery from spinal anesthesia can be quick.

No.: You should have no pain after the cervical cerclage is placed. Any pain thereafter would be related to the pregnancy itself and not the suture.

No: But you really should make sure its ok by your doctor as asex causes contractions, and if you just had this done why push your luck.

ASK THE DOC WHO: Did your surgery, all? S of this nature need to be asked to the doc who put your cerclage in as that doc knows the condition of your cervix, I do cerclages and I give all those instructions to my pts both before and after the surgery so they know everything before they go home so call your doc even if it is the weekend or holiday, sometimes u can and sometimes u can't so please ask your doc.
