Torn toenail: The best way to treat a torn toenail is to keep it clean, apply an antiseptic, and place a dressing over that will protect it. The problem is that it will take approximately one year for the nail to grow and replace the torn portion with a new nail, especially if the nail is torn back to its growth center. Toe bone injury.: It sounds as though that "mound of skin" may be the result of injuring the toe bone just under the nail bed. Occasionally, the bone, when injured responds by a sudden growth spurt. As it grows upward, it raises the nail bed up and can cause a nail distortion as you have described. An r-ray will usually confirm the event. The treatment involves removing the boney spur. Mycotic nails: Actually 8 months of therapy for a fungus nail involving the great toes is not a long time since it takes about a year for the nail to completely regrow. Topical treatments however have limited benefit. Oral antifungals like Lamisil (terbinafine) have produced better outcomes. Current literature supports pulse dosing over the course of a year. ...Read more

Causes of pain may include: trauma (fracture, sprain, arthritis), infection (ingrown nail), metabolic conditions (gout), musculoskeletal or biomechanical abnormality and neurologic conditions (neuropathy). Depends: If there is not some pathological underlying cause that caused the nail to fall off and needs treatment, it is usually ok to wait. The nail will ultimately grow back but it can take even up to about three months to do so. ...Read more

Sometimes it lifts up only partially, and is still fixed in some part.
