Need specialist eval: Your complaints of pain would not be due to otosclerosis. You need an ear nose and throat specialist evaluation to determine the cause of your pain. I have flaky itchy akin in ears, I wear hearing aids in ears and I have itchy scalp. My doctor prescribed otocomb otic ointment for ears no change.
A different drop?: I am not the biggest fan of preparations that contain neomycin as it is known to irritate the skin of some individuals, making the itching worse. Generally complete avoidance of q tips and using a different drop like dermotic (there is a generic now too) twice a day as needed can help. What: What you are describing sounds like pulsatile tinnitis. Before recommending treatment, it would be reasonable to reconnect with your ENT to explore possible causes that include certain vascular tumors of the head and neck (i.e., paragangliomas), atherosclerosis that may be creating narrowing and turbulent flow of your carotid artery (which runs close to your middle ear), possible avm (arteriovenous malformation), etc. After a thorough head and neck exam, and audiogram, he/she may wish to get a contrast ct or mri/mra study. Ask ENT why: Did your ENT do tympanograms? These show whether there is conductive hearing loss from fluid. You are right that perhaps with resolution of fluid, your hearing may improve.
