Think rheumatic dz: Definitely confusing! Nontraumatic IP joint swelling, especially in the thumb, brings to mind rheumatoid arthritis (RA). A common complication of RA is costochondritis which affects the rib joints (chest pain) & a variant called Caplan's Syndrome leads to lung nodules. Unlikely HIV: These aren't common ways for ARS. The could be uncommon causes of end-stage HIV but even then it's not common to see just these symptoms. Muscle twitches and atrophy and according weakness of the affected side could be due to early age polio, impinged nerve in the spine or other nerve disease. Sounds systemic: There are too many problems to be isolated to 1 specific part of the body. I am not sure of what medications you are taking or what other medical conditions you have, but given your age I suspect that you have an electrolyte imbalance that is causing this cascade of problems.
