Facet, disc, nerve: You may have irritated a higher cervical facet joint. Less likely you could have a cervical nerve root irritation or an occipital nerve irritation. Try gentle heating pads, ice, range of motion of your head and neck, gentle stretching, and go see your doctor to see if physiotherapy, medications like antiinflammatories, anti-spasmodics, nerve medications or injections may help you. Nondiagnostic: Without a thorough history and physical examination of the area, the problem is nondiagnostic at this time. However an irritable nerve or muscle may be causing the discomfort. If a rash appears shingles is a possibility. If symptoms persist or worsen see your physician. Unlikely: Unlikely "serious" like life threatening since you've had same symptoms for a few years. I would see a TMJ specialist to rule in or out TMJ or occipital neuralgia.

Neck causes of pain: Much more history is needed to diagnose your problem. Sudden onset, fever/chills, or vision changes are serious and need immediate evaluation. Once those are ruled out, many headache causes come from the neck, called "cervicogenic headaches". Xrays/mri/physical exam/fluoroscopic injections can diagnose the problem. The scm muscle can cause headaches and ear pain.
