What is the difference between off-pump and on-pump coronary artery bypass?

Controversial: On pump - not moving heart better grafting condition; postop graft latency may be better; more postoperative neurocognitive dysfunction; more bleeding and transfusions. Off- moving heart with maybe less postop patency of the graf (skill dependent); less blood loss and transfusions; less neuro problems; less renal problems. Mortality the same. Depend on skill, comfort of surgeon and hospital.

My lower back hurts after excercising (body pump class). How long shall I not go to the class, n what's a good cream (tiger balm?) to use?

Pain free x 2-3 days: You shouldn't return to exercise until you've been pain free with activities of daily living for at least 2-3 days. Then, do so gradually and avoid painful exercises. I don't find over-the-counter topicals very effective, especially for low back pain where the source of pain is rarely very superficial. Oral anti-inflammatories tend to work more effectively.

As long as U eat a-: -balanced diet, exercise cardio & strength on alternateing days U should B a lean 138. If U seem 2 B carrying some fat that U would like 2 loose, strength & cardio along with adequate protein intake, the fat will gradually B gone. The exchange of muscle 4 fat means U may not loose weight, but Ur muscles will B more defined & U will look great.

Omnipod insulin delivery system versus other insulin pumps, what's the difference?

Patch pump: Hi. Omnipod is a "patch pump", meaning the pump attaches directly to your skin every 3 days, and there is no tubing attaching the pump to the infusion site like there is in traditional insulin pumps. There are plusses and minuses to both types of pumps, and neither type does anything for control you can't do with multiple daily injections using analog basal and bolus insulins. CGM is more vital.

HR and EF: Most likely your heart rate was 100 and your ejection fraction (what percentage of blood volume your heart ejects per beat) is 66%. The later is normal. Your HR is elevated because of the atrial fibrillation. This can be further controlled with medicines if symptomatic. Consider a further discussion of atrial fibrillation and its implications with your cardiologist.

Very complicated.: Even after a lot fo studies, it is controversial. It depends on a lot of things. Some people have aortas that cannot be clamped or manipulated due to calcium. Off-pump bypass may be their only option. Other patients can't have off pump bypass. For most people, the decision should involve the patient, the surgeon, and other trusted physicians.

It's not water: What looks like water is actually saline, designed to be similar in electrolyte constitution to blood. It provides a replacement of lost fluid until blood can be obtained. Blood takes a while to obtain and may not be needed if the bleeding can be controlled. That is why saline fluids are used first.
