Varicocele : It could be due to an abnormal collection of veins in the scrotum. An ultrasound should be able to tell if that is the cause. The other possibility is a hydrocele, which is a fluid filled cyst in the scrotum. A referral to a Urologist may help with the diagnosis and treatment. ...Read more

Cervical Stenosis?: The doc on the scene would know more; however, based on what you have written, it sounds like you may have cervical stenosis. The basic idea is that the cervix won't open so menstrual blood can't get out and collects in the uterus. This is painful. Probably not related: Genital pain can be confused but usually still can be confidently diagnosed with a detailed focused history, physicals on scrotum and its contents, and tests as needed after analyzing the quality and quantity of pain with its detailed sequence of events. Not likely: Although it is possible, that is not the most likely diagnosis. Torsion is mostly very sudden and very painful and is an emergency. If you are concerned, get examined asap. Even incomplete torsion left unattended can result in testicle loss. ...Read more

See your provider: This sounds like a normal size uterus and normal size ovary. Endometriomas are ovarian cysts which contain old blood in them. 18 mm is pretty small. If you haven't had it, you might still benefit from a laparoscopy to look for and treat endometriosis especially if you have 4 years of infertility. You might also need reproductive endocrinologist (infertility specialist) if you haven't already. ...Read more
Rare: This is a rare complication <1%. Can be sudden and painful occuring in the first few days. You get +++swelling. Many urologists will suggest surgical drainage to speed up recovery, but waiting for your body to absorb the blood is an option too... Though you might get frustrated with this. Excess fluid: Testicular cancer is in the testicle itself. Almost all things found outside of the testicle are benign.
