PAIN: I believe that if you had necrosis of the testicle you would have pain, fever and generalized illness. If you have some mild discomfort and swelling on the back of the testicle it may be epididymitis. If you have any lump on the testicle you must get this checked by your doctor. ...Read more

Intermittent torsion: Is a possible explanation, could be confirmed if you twist testicle counterclockwise till painful. Probably intermittent torsion if twisting exactly reproduces pain.
