Couple of reasons: First of all you may have an infection that is not picked up on a test (so theoretically he could still be positive). Also sometimes the medicine does not work. Not necessarily: It's common to find chlamydia in only one person in a couple. False pos tests are rare; probably your test is accurate but bf's tests maybe false neg. Or you had it first and he was not yet infected. Or he had chlamydia that cleared before he was tested. More info needed: Something doesn't compute. Nearly impossible for a virgin (no penile-vaginal sex) to have chlamydia, and also unlikely your boyfriend gave it to you. His negative test makes sense, but your positive one doesn't. OTOH the standard nucleic acid amplification tests (NAAT) are very accurate. Rx fail, reinfection: This isn't rare. 5-10% of people with chlamydia have new positive tests after 3-6 months. That's why retesting is recommended. Either you are reinfected (if sexually active and potentially exposed) or treatment failed, which happens 2-3% of the time with azithromycin. Your current partner (s) need to be tested and treated, and you need re-treatment, probably with doxycycline, not azithro. ...Read more

Probably yes, but...: If you had symptoms at the time (usually a discharge from the penis, with or without mild discomfort urinating), and they have cleared up, probably you were cured. However, the treatments are not perfect, and everybody with chlamydia is recommended to have a follow-up lab test (urethral swab or urine) 3-6 months after treatment. What type of test?:
Did he get treated? It is likely that he had infection and was treated effectively and is no longer carrying the organism. He may still have antibody to chlamydia but not the organism. It would not do any harm to get retested by the Chlamydia nucleic acid. See this site for info.

He must be treated: Men do have a higher false negative rate then women. However, much more common, men will tell their partner they didn't have it because they want to deflect any suspicion they are having sex outside of the relationship. Quite common: This happens a lot. Your partner was infected but his immune system cleared it before testing. Or he took an antibiotic (maybe for something else) and it cured him before testing. You had it from distant past partner and current partner was lucky and didn't catch it. The tests miss some infections. Or he just wasn't telling the truth. Probably not if...:
The 4 pills totaled 1000mg of azithromycin. Typically that regiment is recommended by CDC and has a failure rate of under 3%.
If you have a vaginal discharge and/or fever, you should have that checked out regardless of what you took in the past.
