Why would u hurt in ur back at lower shoulder blade, it feels like a pin or a knife sticking me in that spot?

A few things : A few thing can cause this. Zoster in its early stages can do this before seeing shingles. A pleuro-pulmonary process, nerve pain from the intercostal nerves may as well. Muscular etiology less likely. A "pinched" nerve in the lower part of the neck or the upper back quite commonly manifests this way. Start by seeing your doctor for an exam and x-rays. ...Read more

Look for a diagnosis: Why stay with a treatment that is obviously not working . First, is there a clearly established diagnosis & if your shoulder blade or scapula is painful and is out of place or winging you may have a long thoracic nerve issue that can be evaluated by nerve testing. Mri of neck and or shoulder may also be of benefit. See an orthopedic surgeon, physiatrist or neurologist for further evaluation. ...Read more

Pleuritic catch: Several possibilities. The pain may be related to irritation of the pleura (lining of the lung) or ribs or muscles around the scapula. A chest x ray would be a good start but may not show any abnormalities.
