Tight hamstring?: Do you sit a lot? Do you drive a lot? It sounds like there was no injury and there is nothing abnormal in the area to your naked eye. Plantaris muscle : The plantaris muscle starts just above the back of your knee and goes down to your Achilles tendon at the heel. It is right in the middle of your calf. It doesn't stretch well. Strain/sprain: It is likely a strain or sprain with possible tendon tear. Inadequate stretching before strenuous exercise or some type of fall may have caused some damage. If there isn't significant swelling and bruising, he should follow the r.I.C.E protocol. Rest, ice, compression and elevation. Still could be Baker: If it is painful it needs further evaluation. A bakers cyst should easily be seen by ultrasound but sometime the content of the cyst, tissue, synovium, blood, can obscure its deliniation. If x-rays have been ok you may need an MRI but need to consider vascular structures both vein or artery, the posterior knee, cartilage, tumors, etc these are the things to look for. ...Read more
