Not necessarily: SV="supraventricular rhythm" which is normal. PVCs are "extra beats" that sometimes lead to the feeling of a thump or irregular heart beat. By themselves, extra PVCs or couplets of PVCs are not dangerous. Some people get them when they drink caffeine or smoke cigarettes. Talk to cardiology: I am assuming that you have symptomatic bradycardia, and that was the reason you had a Holter study done. Now, I don't know how long your HR stayed at 27, but even 40 is too low. Unlikely: With a normal heart based on all the studies you have had, PVCs are usually benign and not like to precipitate ventricular tachycardia. The caveat is that you are on diltiazem as well as amitriptyline both of which can affect the heart rhythm. Thus you should continue to have your EKG monitor. All the best to you! ...Read more
