No...actually: The zn stain is a broad stain for acid fast bacilli of which mycobacterium tuberculosis is one of many. The specific tests like quantiferon or the pcr test are against you having tb. The culture is the "gold standard" but you will need to wait for it. It might turn out be a cousin of TB and that too will grow out of culture. Treat it: The positive H. pylori antibody test is not a reliable indicator of current infection. The gold standard for H. pylori treatment is the gastric biopsy, and this is negative in this patient. However, I am impressed by the positive breath test and erosive gastritis. I would have expected stool antigen to be positive, as it mirrors the breath test. Contradictory Tb tst: Quantiferon-TB(the blood test) and the PPD(skin test) are not recommended together. False positive test: Yes you can have what is called a "false positive test." This means that at one point you where exposed to TB, you where in close contact with someone. The usual next step is to have a chest X-Ray to see if you have TB in your lungs. A false positive test is quite common and nothing need to be done. Not at 15cm, but...: The bump is more likely 15mm, because 15cm is 6 inches and HUGE. A 15mm bump on a screening test is big but may still be a false positive (a screening test means the patient has NO symptoms of tuberculosis). If a patient has contacted somebody with TB, or has current symptoms, she likely has a true positive unless her bump is due to past BCG vaccine.
