Tapering Hypokalemia Of Prednisone
TBI pt titrating down with prednisone. Steady increase in depression with decrease in prednisone dose. Now at 13mg. Suggestions? Adrenal Fatigue?
Referrals: W/o prior adrenal disease, only lengthy, high dose prednisone could result in them underperforming and leading to depression. It's more likely reactive depression and a Psych Eval may help. So may followup by neurologist from TBI. A course of therapy w/ Health Psychologist may help adjust to injury/aftermath. ...Read moreSee 2 more doctor answers
Prednisone is a synthetic cortisone. The body makes cortisone, a natural hormone made in adrenal glands. The body converts it to Hydrocortisone to become active. 25 mg of cortisone has about same effect as 5 mgm prednisone. The average person would produce 3-6 mg of pred daily. So why use a substitute? The synthetic has more anti-inflammatory effect; but has less effect ...Read more

Started having interdose withdrawals with 2 mg of Ativan divided dose. --dr switched me to 1 mg of klonopin (clonazepam) divided dose ..does this make sense?
Yes: Ativan 2mg is the maximum dose that can be safely given at any one time, so this places patients at greater risk of uncomfortable withdrawl symptoms until the next dose is administered. Furthermore, Klonopin (clonazepam) is easier to manage and tolerate than Ativan because it has a longer half life, or a longer time that its chemicals remain effective in the human body. ...Read moreSee 1 more doctor answer

Any major risks with a combo of 300 mg of bupropion, 20 mg of escitalopram and adding 20 mg of omeprazole? risk of serotonin syndrome or seizure?
Theory yes Serotonin: Omeprazole is a MILD blocker of liver metabolism (and 20 is low dose). Those are average doses of two antidepressants. They have a WIDE margin of safety. So, serotonin syndrome (two antidepressants both raising serotonin) or seizures (too much welbutin) is a conceivable concern. But it sure isn't likely. Unless there's something else to cause seizures or other serotonin drugs. ...Read moreSee 1 more doctor answer
Pdoc set taper of seroquel (quetiapine) with alternating dose of 50 mg & 25mg for several weeks, is this right way of taper and what's rationale with this method?
Seroquel (quetiapine): Not sure. Are these regular seroquel (quetiapine) or seroquel (quetiapine) XR? Maybe your doctor was trying to do the equivalent of a taper from 50 mg to 37.5mg to 25 mg. Although it wouldn't be my choice of how to taper - it is one option. Depending on which dosage & forms you had - maybe your Dr. was trying to save you $ by using up the left over pills that you already had. With complex meds - consider a psychiatrist. ...Read more
What is proper increments to wean from 6 mg of risperdal while Latuda (lurasidone) starting dose of 40 mg to lessen psychotic withdrawal,dx bipolar 1w/psychosis?
Switch over caution: There is more than one way to switch over from an antipsychotic mood stabilizer to a new one. All depends on how well the new one starts to work on your symptoms, if it is tolerated, or if produces any side effects. Also, it is important to consider for how long you have been in the previous one, to make sure there is no risk of return of symptoms in a short period of time. ...Read more

Yes but: Your medical regimen is becoming complicated and you risk encouraging a pattern of self-medicating. While there is an important place for medications in helping brains function better and people feeling better, be careful not to fall into the "chemical coper" trap. Discuss this with prescriber. I suggest having only one. ...Read more
