My brother in law got sudden dizziness, then followed by nausea, now shaking. We took him to the ER and we still have no answers. What could this be?

Those sound like: Anxiety. They are possibly present in real diseases, but not likely w/out other symptoms (or less visible signs). Vertigo: Definitely sounds vestibular. You might have just developed Meniere's disease of the inner ear with acute 'attacks.' More likely with ringing and hearing loss in one ear, but not necessarily. Or it may just be 'benign positional vertigo' on top of the general dizziness you've been having. My own take on this is that, particularly with heavy menses, there is notable amount of retrograde menstrual flow (back flow of menstrual effluent) through the fallopian tubes into the abdominal cavity.
