Should: Not really be that painful. I assume the pin was removed after several weeks. By now the healing process should have been well underway. Is what it sounds like. A small area like a toddlers toe will swell impressive. My favorite treatment for insect/ spider bites is a raw onion... Slice it and rub on the affected bite area tl the juice is dry (a couple minutes). It neutralizes the toxin and reduces swelling.

It sounds like the second toe may be contracted at the joint. The most likely causes for a second toe being hammered are long second toe and a bunion. So if the second toe is longer than the great toe then the shoe hits the tip of the toe causing it to contract.
