But it could also be a "sentinel node" which might be an early manifestation of lymphoma. Swollen lymph nodes in the neck from infection usually come up quickly and are tender. May be neck muscle: If it is really behind the neck, it isn't likely to be a gland. The glands are mainly on the front side of the neck. One of the larger back of the neck muscles may be larger than the other side, or may be swollen from being over used. A: A mass present for a month, located under the earlobe, and unresposive to antibiotics may very well be a parotid tumor (the majority of which are benign). I would see an ENT physician who should be able to distinguish this for you, and most likely will be able to offer to you fine needle biopsy for tissue diagnosis.
