If the doses are not equivalent, then you could have recurrence of symptoms or you could have side effects from too high a dosage. 3-7% of the population cannot make the switch easily due to different metabolic rates of these two medications. Most discomfort people experience in switching from one ssri to another is due to discontinuation from the first med, if the second one isn't at a sufficient level to avoid those discontinuation effects. Not necessarily: As long as you take a dose of zoloft which is equivalent to the Lexapro (escitalopram) dose, the switch should be okay. Maybe you can discuss this with your prescribing physician as you shouldn't make adjustments on your own. Finally, and unlike lexapro, (escitalopram) zoloft may be better absorbed (you get more), when taken with food-perhaps up to 50% better absorbed with food. Good luck. ...Read more

Cross titration: Zoloft and Lexapro (escitalopram) are both metabolized by the same pathway. As such, a cross titration is usually recommended to avoid any adverse effects. As a side note, that's a pretty low dose of Zoloft -- so you might not achieve an adequate response. If this happens, don't hesitate to increase the dose (with your doctor of course). Start Lexapro (escitalopram): While still on the zoloft. I usually have someone start on a small dose of the Lexapro (escitalopram) and increase it gradually while at the same time, taper down even more slowly on the zoloft. Each new antidepressant essentially has to start over again and needs time to work. Therefore you don't want to go off the zoloft too quickly, you may find out it had been doing much more for you than you had thought. ...Read more

Generally yes: Obviously you should talk to your prescribing doc, don't switch behind their back. They need to know what you're taking, & may have other options to suggest. But yes, it's usually safe & easy to switch between Lexapro (escitalopram) & zoloft. Note that the dosages (mg) are different. Also, zoloft is available as generic while Lexapro (escitalopram) isn't, so Lexapro (escitalopram) costs more & insurance may not cover it. ...Read more

Transition: Everyone responds differently to going off ssris, and some people are fine with one and not with another. Both of the meds you are talking about selectively inhibit serotonin, so in essence, you are doing the same thing with one medication as the other, so you are really not going off anything cold turkey. Yes: It's reasonable to take the Lexapro (escitalopram) at the same time of the day that you would normally take your Zoloft the next time you are due for a dose. You just should not take both drugs on the same day, and ideally you will not skip a day either - best just to go from Zoloft one day to Lexapro (escitalopram) the next. The Ativan will not affect the switch from one drug to another either way. ...Read more

Zoloft to Lexapro (escitalopram): Many physicians will do a cross taper gradually lowering the dose of one antidepressant while raising the dose of another. If however it has been determined that the zoloft has been of absolutely no benefit than there is no need for a cross taper. The Lexapro (escitalopram) should block any possible ssri discontinuation syndrome. Discontinuation symptoms with zoloft have been reported to be 1 to 2% but in my clinical experience have actually been far less than that. You should have no problem doing this. Not much: Both are selective serotonin reuptake inhibitors, or ssris, a fancy way of saying they help your brain hold onto and use more of the serotonin it has. Serotonin is one of the chemicals in your body related to happy things, like food, sex, and sleep (which is why the side effects often affect these issues).
